Job Summary:

The Nutrition Education Coordinator is responsible for the nutritional assessment, development of a dietary plan and nutrition education of the cardiac rehabilitation patient. The plan includes the identification of dietary recommendations to improve coronary risk factors and promote the consumption of a nutritionally adequate diet. The Nutrition Education Coordinator responsibility also includes community education.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job.

* Completes and documents a nutritional assessment of cardiac rehabilitation patients/participants including dietary history, and nutritional and age related needs.

* Develops a nutritional plan for cardiac rehabilitation patients and their families as well as auxiliary program participants and referrals.

* Develops a teaching plan individualized for each patient.

* Plans and implements nutritional programs including grocery tours and cooking demonstrations.

* Provides education on dietary modifications to reduce cardiovascular risk, maintaining dietary restrictions while eating in restaurants, weight loss options and other related areas.

* Evaluates patient response to intervention.

* Demonstrates organization skills in providing patient care.

* Exhibits a professional practice, using positive interpersonal skills.

* Demonstrates positive customer relations.

Minimum Education, Training and Experience Required:

A. Current Registration by American Dietetic Association.

B. Masters degree in foods, nutrition, or other allied health profession, preferred.

C. Bachelors in dietetics, nutrition, or foods and nutrition and an American Dietetic Association approved internship in a hospital practicing therapeutic dietary counseling in cardiovascular clinical area.

D. Twenty-four months recent and relevant experience.
