Job Description:
Coordinating treatment plans for individual patients, including: evaluation, consultation, education and follow-up services.
Identifying services available in the community that could potentially benefit patients, arranges for delivery of those services, and assists patients in gaining access to services, including scheduling appointments.
Conducting psycho-social evaluation upon admission. Providing psycho-social education to patients or their family members to increase their understanding of the patient's illness.
Participating as an inter-disciplinary team member on the unit as it relates to treatment planning, treatment reviews and discharge planning.
A Master's degree in social work or a related field
Two years of experience
Basic knowledge of mental and physical characteristics of persons with serious mental illness;
Good communication and interpersonal skills;
Ability to manage case load with rapid patient turnover.
